As a registered member of the College of Psychologists of Ontario I adhere to strict ethical guidelines and standards of professional conduct. As such, I respect you as client and I am committed to keeping your personal information confidential. I require written express consent to release your personal information. There are some legal and ethical limitations to confidentiality that may require the release of information with or without your consent. Some examples of this include:
If you are assessed to be in immediate danger of harming yourself, or at risk of seriously harming someone else, I am required to take appropriate action to protect your life or the intended victim (e.g. notifying police, emergency contact or person under threat).
If there is reason to believe a child is in need of protection, or apparent, suspected or potential child abuse or neglect, I am required to contact the Children’s Aid Society.
If there are grounds of apparent, suspected or potential elderly abuse in a retirement home or long-term care facility, I am required to notify the Director of the Ministry of Health and Long-Term Care and the appropriate regulatory body.
If you report past or current sexual abuse by another regulated health professional (e.g. doctor, dentist, chiropractor), I am required to notify the appropriate regulatory body.
If a Court of law subpoenas your psychology file, I am required to release any Court ordered information.
If the College of Psychologists of Ontario conducts a random audit of my services to ensure the quality of services being provided, I am required to give access to the assessors who are also bound by ethical standards of confidentiality.
